- Personal Identifiers: Full name, phone number, email address, date of birth.
- Medical & Health Data: Symptoms, consultation notes, appointment history, prescriptions (uploaded voluntarily).
- Payment & Transaction Data: Razorpay payment ID, appointment fees, partial card details.
- Technical Data: Device ID, IP address, app version, crash logs.
2. How We Use Your Data
- To facilitate doctor appointments and teleconsultation services.
- To process payments securely via Razorpay.
- To improve user experience and provide customer support.
Terms & Conditions
These Terms govern your use of Swasthya Plus App. By downloading, registering, or using the App, you agree to be bound by these Terms.
1. Eligibility & User Account
You must be at least 18 years old to use the appointment booking service. You are responsible for maintaining confidentiality of your login credentials.
Refund & Cancellation Policy
This policy applies to all paid appointments and consultation fees processed via Razorpay. Please read carefully before booking.
The Only Exception for Refund
A refund will be initiated ONLY if the visiting doctor fails to attend the scheduled appointment on the assigned day and cannot provide a rescheduled slot within 15 days.
